Sejarah Farmasi Dunia

Sejak masa Hipocrates (460-370 SM) yang dikenal sebagai “Bapak Ilmu Kedokteran”, belum dikenal adanya profesi Farmasi. Seorang dokter yang mendignosis penyakit, juga sekaligus merupakan seorang “Apoteker” yang menyiapkan obat. Semakin lama masalah penyediaan obat semakin rumit, baik formula maupun pembuatannya, sehingga dibutuhkan adanya suatu keahlian tersendiri. Pada tahun 1240 M, Raja Jerman Frederick II memerintahkan pemisahan secara resmi antara Farmasi dan Kedokteran dalam dekritnya yang terkenal “Two Silices”. Dari sejarah ini, satu hal yang perlu direnungkan adalah bahwa akar ilmu farmasi dan ilmu kedokteran adalah sama.

Dampak revolusi industri merambah dunia farmasi dengan timbulnya industri-industri obat, sehingga terpisahlah kegiatan farmasi di bidang industri obat dan di bidang “penyedia/peracik” obat (=apotek). Dalam hal ini keahlian kefarmasian jauh lebih dibutuhkan di sebuah industri farmasi dari pada apotek. Dapat dikatakan bahwa farmasi identik dengan teknologi pembuatan obat.

Pendidikan farmasi berkembang seiring dengan pola perkembangan teknologi agar mampu menghasilkan produk obat yang memenuhi persyaratan dan sesuai dengan kebutuhan. Kurikulum pendidikan bidang farmasi disusun lebih ke arah teknologi pembuatan obat untuk menunjang keberhasilan para anak didiknya dalam melaksanakan tugas profesinya.

Dilihat dari sisi pendidikan Farmasi, di Indonesia mayoritas farmasi belum merupakan bidang tersendiri melainkan termasuk dalam bidang MIPA (Matematika dan Ilmu Pengetahuan Alam) yang merupakan kelompok ilmu murni (basic science) sehingga lulusan S1-nya pun bukan disebut Sarjana Farmasi melainkan Sarjana Sains.

Departemen Tenaga Kerja Republik Indonesia (1997) dalam “informasi jabatan untuk standar kompetensi kerja” menyebutkan jabatan Ahli Teknik Kimia Farmasi, (yang tergolong sektor kesehatan) bagi jabatan yang berhubungan erat dengan obat-obatan, dengan persyaratan : pendidikan Sarjana Teknik Farmasi.

Buku Pharmaceutical handbook menyatakan bahwa farmasi merupakan bidang yang menyangkut semua aspek obat, meliputi : isolasi/sintesis, pembuatan, pengendalian, distribusi dan penggunaan.

Silverman dan Lee (1974) dalam bukunya, “Pills, Profits and Politics”, menyatakan bahwa :

1. Pharmacist lah yang memegang peranan penting dalam membantu dokter menuliskan resep rasional. Membanu melihat bahwa obat yang tepat, pada waktu yang tepat, dalam jumlah yang benar, membuat pasien tahu mengenai “bagaimana,kapan,mengapa” penggunaan obat baik dengan atau tanpa resep dokter.

2. Pharmacist lah yang sangat handal dan terlatih serta pakart dalam hal produk/produksi obat yang memiliki kesempatan yang paling besar untuk mengikuti perkembangan terakhir dalam bidang obat, yang dapat melayani baik dokter maupun pasien, sebagai “penasehat” yang berpengalaman.

3. Pharmacist lah yang meupakan posisi kunci dalam mencegah penggunaan obat yang salah, penyalahgunaan obat dan penulisan resep yang irrasional.

Sedangkan Herfindal dalam bukunya “Clinical Pharmacy and Therapeutics” (1992) menyatakan bahwa Pharmacist harus memberikan “Therapeutic Judgement” dari pada hanya sebagai sumber informasi obat.

Melihat hal-hal di atas, maka nampak adanya suatu kesimpangsiuran tentang posisi farmasi. Dimana sebenarnya letak farmasi ? di jajaran teknologi, Ilmu murni, Ilmu kedokteran atau berdiri sendiri ? kebingungan dalam hal posisi farmasi akan membingungkan para penyelenggara pendidikan farmasi, kurikulum semacam apa yang harus disajikan ; para mahasiswa bingung menyerap materi yang semakin hari semakin “segunung” ; dan yang terbingung adalah lulusannya (yang masih “baru”), yang merasa tidak “menguasai “ apapun.

Di Inggris, sejak tahun 1962, dimulai suatu era baru dalam pendidikan farmasi, karena pendidikan farmasi yang semula menjadi bagian dari MIPA, berubah menjadi suatu bidang yang berdiri sendiri secara utuh.rofesi farmasi berkembang ke arah “patient oriented”, memuculkan berkembangnya Ward Pharmacy (farmasi bangsal) atau Clinical Pharmacy (Farmasi klinik).

Di USA telah disadari sejak tahun 1963 bahwa masyarakat dan profesional lain memerlukan informasi obat tang seharusnya datang dari para apoteker. Temuan tahun 1975 mengungkapkan pernyataan para dokter bahwa apoteker merupakan informasi obat yang “parah”, tidak mampu memenuhi kebutuhan para dokter akan informasi obat Apoteker yang berkualits dinilai amat jarang/langka, bahkan dikatakan bahwa dibandingkan dengan apotekeer, medical representatif dari industri farmasi justru lebih merupakan sumber informasi obat bagi para dokter.

Perkembangan terakhir adalah timbulnya konsep “Pharmaceutical Care” yang membawa para praktisi maupun para “profesor” ke arah “wilayah” pasien.

Secara global terlihat perubahan arus positif farmasi menuju ke arah akarnya semula yaitu sebagai mitra dokter dalam pelayanan pada pasien. Apoteker diharapkan setidak-tidaknya mampu menjadi sumber informasi obat baik bagi masyarakat maupun profesi kesehatan lain baik di rumah sakit, di apotek atau dimanapun apoteker berada.
